Schedule an Appointment:

Click on the "Schedule Appointment" link.

Select "telemedicine" and the day and time that works for you.


You will receive via email patient intake forms; please complete prior to your appointment.

Your Appointment:

Leading up to your appointment, you will receive several reminders. When it is your appointment time, please join with a device that has a camera.  


What to Expect:

In your 30-minute appointment, the provider will review your history, assess your current symptoms, and address any questions you may have before recommending any treatments, care plans, etc. This may involve prescribing medications, sending you information, and recommending a regimen to treat your illness. 

Changes to your Appointment:

If for any reason you need to change your appointment, you can contact us simply by responding to the text message or email you receive when you scheduled your consult. We will make reasonable accommodations to change or cancel your appointment, however, if you fail to enter your telemedicine session without notice, we reserve the right to charge you for your missed appointment.
