iCare was created to make high-quality health care accessible to individuals and families across Idaho. Founded when telehealth was still emerging, iCare provided support for acute needs helping families access care when in-person options were limited or unavailable.
As iCare grew, many patients were seeking more than episodic care. Families wanted continuity, thoughtful assessment, and ongoing support particularly for mental and emotional health.
Today, iCare provides accessible mental health and family-centered care through secure telehealth, meeting patients where they are and partnering with them over time.

iCare is proud to be Idaho-based and dedicated to improving access to care throughout the state. Through telehealth, we are able to reach patients in both rural and urban communities who may otherwise face long wait times or limited local resources. We accept Idaho Medicaid and most major commercial insurance plans and strive to reduce barriers that often prevent individuals and families from seeking care.

Patients can expect a supportive, respectful experience focused on understanding their concerns and developing a clear plan of care. Appointments are conducted through secure telehealth in a private, confidential setting. When appropriate, our providers may also address select acute or medical concerns within the context of comprehensive, ongoing care.
Meet Your Providers

Kelly Graff
Kelly Graff, MSN, APRN-CNS is an advanced practice Clinical Nurse Specialist in Acute Care Pediatrics providing telehealth services through iCare Telecare. She earned her Master of Science in Nursing from the University of Delaware and completed her undergraduate nursing education at Boise State University.
As a Clinical Nurse Specialist (CNS), Kelly is uniquely trained in research and evidence-based practice, allowing her to translate the latest clinical research into thoughtful, individualized care. Her clinical focus includes youth, adolescents, and young adults, with an emphasis on pediatric care and recovery-oriented services.
Kelly is currently completing her Psychiatric Mental Health Nurse Practitioner (PMHNP) training at South College, with specialized focus in pediatric mental health and recovery services. Raised in Idaho, she brings a strong understanding of the needs of local families and communities.

Lisa Leazenby
Lisa Leazenby, DNP, PMHNP, is a board-certified Psychiatric Mental Health Nurse Practitioner with over 24 years of clinical experience providing compassionate, evidence-based telehealth psychiatric care.
She offers psychiatric medication management and supportive psychotherapy for adolescents and adults, treating conditions such as anxiety, depression, ADHD, mood disorders, trauma-related concerns, and life-transition stress. Lisa takes a collaborative, patient-centered approach, creating individualized treatment plans that focus on both symptom relief and long-term stability.

Lisa holds a Doctor of Nursing Practice degree and is dual-certified as a PMHNP and Family Nurse Practitioner, allowing her to consider both mental and physical health when developing care plans. All services are provided via secure telehealth, offering convenient, accessible care while maintaining a strong therapeutic connection.
Lisa is licensed in Idaho and other approved states.
